Disability Services at Latrobe Community Health Services

Disability Services provides an information and referral service.  We can assist you in planning your future support needs including transitional periods in your life such as leaving school or moving out of home. We support carers to receive respite so they can continue to provide the best possible care. We understand that independence is your ultimate goal with you being in control of your life at all stages.

The word ‘partnership’ best describes the relationship between you and Disability Services. Our support is self-directed which puts you in the driver’s seat of decision making regarding all aspects of your life including your care and support needs. We will assist you to navigate the ‘system’ so you can reach your goals.
